濕潤傷口癒合觀念及傷口床準備觀念

    20世紀50年代後期人們在研究中發現,傷口環境對傷口癒合起著至關重要的作用,其中有兩個重要的發現:奧蘭(Odland) 1在1958年首先發現被保持完整的水泡的傷口比水泡破裂的傷口癒合的速度要快;英國倫敦大學的溫特(George D. Winter PhD (1927-1981))2-4在1962年Nature雜誌上發表濕潤癒合的概念,他以豬皮做實驗,以高分子薄膜(polymer film,一種軟質的塑膠防水薄膜)緊蓋保護傷口,發現『傷口在適度濕潤的環境下其細胞再生能力及游移速度較快,其復原速度比在完全乾燥的環境下快一倍以上』。喜門(Hinman)及美巴克(Maibach)5在1963年報導了同樣的實驗結果,這兩個重要的發現標誌著濕潤環境合理論的誕生。到了1970年代,半透明片傷口敷料(transparent film dressing)開始出現(但是不透氣),且在1970年洛比6的人體實驗證實了乾淨沒結痂的濕潤傷口上皮細胞移行增生的速度較快,能加速傷口的癒合,於是『濕潤傷口癒合』(moist wound healing, "occlusion" or “moisture-retentive")觀念開始被廣泛接受。

     然而雖然知道傷口保溼的情況可以促進傷口的癒合,但是傷口上面究竟發生什麼事情,基本上上皮細胞本來就都會不斷生長,但為何會有傷口20年不癒? 局部傷口的環境如何被評估及處理以促進傷口的癒合都是尚未解決的問題。一直到1996年Tarnuzzer等人建議在慢性傷口的處理必須注意到生長因子、細胞激素、蛋白質分解酵素等複雜交互作用下的傷口環境7,所以Falanga於2000年提出傷口床準備(wound bed preparation)的概念8,對於慢性傷口處理的概念, 從細胞的觀點思考,利用一些正確的技術去除影響傷口癒合的障礙,加速內在傷口的癒合或便利其他治療處置的有效性,而在2003年,Schultz提出傷口床準備的系統性處理方式9;並在2004年提出臨床上觀察到的四個慢性問題TIME:Tissue傷口組織的顏色及有無壞死,Infection/Inflammation傷口有無感染發炎跡象,Moisture傷口是乾的還是過度濕潤,Edge傷口邊緣的情況有無潛行性口袋,進一步的說明傷口床的四大問題意義,而進一步將它應用於傷口的評估工具10,Smith & Nephew公司正致力於推廣這種觀念。
