燙傷藥膏SSD的歷史

     因為故事有點長,故先講結論,燙傷藥膏SSD( Silver sulfadiazine)出現的背景,在於當時普遍要讓傷口乾燥的年代(1960年代),因為,SSD有著優秀的抗菌能力,故便被廣泛使用在大面積及深層燒傷的患者,但SSD是由水溶液的硝酸銀和磺胺藥合成為親水性的乳霜cream,而cream類的藥用乳膏主要是讓皮膚乾爽,故SSD從來就不是讓傷口保濕,只是後來濕潤療法的觀念崛起,才讓廠商更改使用方法為至少塗抹0.3公分厚,但塗上去的SSD藥馬上就化掉成水了,被紗布吸掉了,又能有多少留在傷口上保濕?尤其是在平面的二度燙傷傷口上,如何保持藥膏仍有0.3公分的厚度。但對於三度燙傷及大面積燒傷傷口,在沒有新的證據出來之前,SSD也許仍是當代的標準使用藥物。最後,但也是最重要的,下面講的,都是嚴重的燒燙傷的治療歷史,而甚麼是嚴重的燒燙傷呢?請詳見文後的附註(台大醫院燒傷中心 住院適應症)。總之就是需要住院的燒燙傷。而其實極大部分我們常遇見的燒傷,如被煙擋管燙到或煎魚被油噴到,並不適用下面的處理。

     好了,故事開始,在很久很久以前,遠在1770年代,人們開始知道使用0.2%的硝酸銀(silver nitrate、AgNO3)溶液來處理傷口。之後出現了各種濃度的硝酸銀,從0.2%到10%,濃度太低無法殺菌,太高又會傷害正常組織。

     但是,一開始,這些藥並不是用來殺菌,而是利用硝酸銀的腐蝕性來讓傷口乾燥,形成凝固狀的焦痂coagulum來減少滲液、避免體液散失,因此許多腐蝕劑(escharotic)便在20世紀初期應運而生,如5%或10%的硝酸銀溶液silver nitrate solution、 龍膽紫gentian violet及tannic acid(單寧酸)等。而在有名的單寧酸在 Davidson1於1925年開始用於燒傷傷口之前,人們對於燒傷的標準治療是讓傷口乾燥,避免傷口的有毒物質釋放出來,等其自然化膿,其並能讓痂皮和其下的組織分離。單寧酸又稱為鞣酸,能使蛋白質凝固。在很久以前人們就知道把生豬皮、生牛皮用鞣酸進行化學處理,能使生皮中的可溶性蛋白質凝固。於是,本來放上幾天就會發臭腐爛的生皮,就會變成了漂亮、乾淨、柔韌、經久耐用的皮革。這種製革工序,叫做皮革鞣製2。所以也就是將人類三度燒傷的死痂皮當作死豬皮來處理,1935年Bettman3將10%的硝酸銀溶液加上2.5%的單寧酸,傷口便能在幾秒鐘內硬化。甚至在二次世界大戰時,美軍對於燙傷的標準治療包括使用單寧酸溶液、磺胺藥抗生素、延遲清創手術、大量輸液及治療骨折4,不過在1944年之後因證實單寧酸有肝毒性後,這個藥才從此不再被使用5

      接著人們開始了解「休克」及「敗血」其實才是導致嚴重燒傷患者死亡的主因6-8。美軍並在1942研究出第一套根據燒傷面積的輸液公式(the NRC First Aid Formula)9-12:在前24小時,每10%TBSA燒傷面積給1000CC稀釋血漿colloid (原本公式是給500ml血漿,但因美國血庫會將血漿稀釋一倍,故患者就會被給成1000ml稀釋血漿) 13,此只適用於面積大於20%的燒燙傷),此公式經過許多改良,目前常用的為 Modified Parkland Formula14:其不用血漿; 改用晶體crystalloid輸液,為4 ml/kg/TBSA,使用Lactate Ringer’s solution輸液,在前八個小時輸入總量的一半,在後十六個小時補充另外一半。而有名的計算燒傷面積的”Rule of nine”則是於1951年才由Wallane發表出來15

     在那時,外科的地位還只是針對三度燙傷患者,等其痂皮分離後,再將之切除,而第一次植皮大約在23天左右16。Artz17在1955年發現說應該慢慢等待,直到痂皮自然脫落,此時在痂皮下方的肉芽組織便已長好,一旦痂皮被拿掉,肉芽組織長好,就應該儘速植皮,而水療可以加速痂皮的脫落及輔助清創18,而屍皮移植可以在自體植皮之前,促進肉芽組織生長19。但這招對於超大面積燒傷(>50% TBSA)的患者卻無效。在1960年代及1970年代,許多醫師嘗試早期移除痂皮,但並無法減少死亡率20。在1940年代那時,雖然有外科醫師嘗試著對三度大面積燒傷患者早期清創(fascial excision,筋膜上切除,清創到露出筋膜),但卻因仍有敗血、照顧問題而無法改善存活率21-24,隨著醫藥及照顧的進步,於70年代Janzekovic25及Jackson26改良手術為早期施行Tangential excision(削果皮式切除,又稱layered excision,以取皮刀將壞死皮膚一層一層的切除,直到出血)加上植皮,然而這早期切除痂皮(7天內)的觀念卻一直持續到1980年代之後,因為證實可改善三度超大面積燒傷患者存活率而才慢慢被接受27,28

     而磺胺鹽sulfonamide抗生素早於1930年代就廣為使用29,自二次世界大戰更廣泛使用於燒傷傷口(磺胺粉,sulfa powder,大量用於1941年的珍珠港戰爭,因為當時也只有這種抗生素可用),並且不會抑制傷口生長,然而在1943年Meleney發表1500名創傷患者的研究30,證明不管是全身性注射型磺胺藥或局部塗抹的磺胺藥粉,對傷口感染都沒有效,且只有充分的手術治療才能控制感染。接著又因為對革蘭氏陽性菌(如Staphylococcus、Clostridium)無效而漸漸被捨棄,取而代之是1942年開始大量生產的盤尼西林31。然而,之後又出現了致命的燒傷併發症:“燒傷敗血癥burn wound sepsis”32,一直查不到原因,直到1964年才由Walker 及Mason等人查出病源是綠膿桿菌Pseudomonas33,34,1964年,Pruitt等人使用mafenide acetate (Sulfamylon磺胺麥龍) cream才大量降低燙傷的死亡率(從59%降至10%)35,此時磺胺藥膏才又被人們所重視。

     至於燒傷的死亡率和2度以上燒傷面積及年紀有關,Baux等人36於1961年發表說「死亡率等於燒傷面積 + 年齡」,稱為怖指數Baux score,於2010年,Osler等人37補充Baux的公式說如果有呼吸道灼傷,則死亡率再加上17%( revised Baux Score: Age + Percent Burn + 17 * (Inhalation Injury, 1 = yes, 0 = no))。現在由於醫學的進步,故死亡率需再打八折,為矯正後怖指數X0.8。如一名50歲男性全身50%2度燒傷並嗆傷,則死亡率為(50+50+17)*0.8=71.6%。

      一直到1965年,Moyer從實驗證實0.5%的硝酸銀溶液是最低可以抑菌的濃度,也發現在該濃度下,對表皮細胞並沒有毒性,並能縮短治療時間38。此時,0.5%的硝酸銀水溶液曾一度是治療燒傷病人的標準溶液。但是硝酸根離子對傷口和人體細胞有毒性,且會降低傷口的癒合速度。於是Charles Fox Jr於1968年將磺胺及硝酸銀混合在一起形成磺胺銀藥膏39(Silver Sulphadiazine, SSD 1%),開始使用在燒傷傷口,其和硝酸銀相比較無腐蝕性。使用硝酸銀的年代,燙傷的死亡率可從90%降到59%,而使用silver sulfadiazine及mafenide則可使死亡率降到5-20%40至今silver sulfadiazine及mafenide仍是燒傷治療最常使用的兩種藥膏

    在1960年代之前,醫護人員會放任傷口上的crust(deep、partial-thickness)或eschar (full-thickness)自然乾燥脫落。雖George D. Winter於1962年就發現濕潤傷口癒合的秘密41,但一直到1970年代人們才終於慢慢接受moist-exposed burns treatment(MEBT)的觀念42

     在1980年代,各種保持傷口濕潤的先進敷料陸陸續續被推出來,如OpSite(1981)43、Duoderm(1982)44、Biobrane(1989)45,mepitel(1990)46,這些敷料雖然沒有殺菌的功能,卻也能保護受傷的皮膚及讓受傷的皮膚保持濕潤的環境,經人體實驗,其竟然比SSD更能促進二度燙傷傷口癒合,並減輕換藥疼痛43-46。有學者認為,原因可能是磺胺銀SSD本身對細胞組織就有毒性,會阻礙傷口癒合47-49,而只單純讓燙傷傷口保持濕潤的方法,即使沒殺菌作用,也會比燙傷藥膏好。

    一直到SSD發明後30年,加拿大的Nucryst公司才於1998年利用Silcryst™奈米銀科技做出第一塊Acticoat™ 抗菌銀敷料,Acticoat主要是在一聚丙烯塑膠膜上利用真空鍍的方式,鍍上一層奈米級銀顆粒所形成的薄膜(Ag 105mg/100cm2)50,並同時證實acticoat是第一個可有效治療二度燒傷的抗菌銀敷料51。之後各式各樣的銀敷料便應運而生,最有名的是2002年發表出來的aquacel-Ag,這些銀敷料主要能持續3-7天連續釋放銀離子,使得換藥頻率可以延長到3-7天換藥一次,降低換藥痛苦,並且比SSD更能促進傷口癒合52-54,而於2008年利用軟性矽膠黏著技術推出的mepilex Ag,更標榜著抗沾黏,除了延長換藥時間間隔外,更加能降低因沾黏導致的換藥疼痛55。故Heyneman於2016年回顧了52篇隨機前瞻性對照試驗的文章56,其比較市面上30種敷料或藥膏,結論是在「不需要開刀的二度燙傷」患者中,既然市面上各式的先進敷料每個都比燙傷藥膏SSD好,並更能加速癒合,SSD已不建議使用在這類小面積二度燙傷患者中。

燒燙傷加護病房住院適應症
1. 大面積燒燙傷

      甲、成人:二度以上燒燙傷面積大於20%,三度以上燒燙傷面積大於10%

      乙、小兒(12歲以下):二度以上燒燙傷面積大於10%,三度以上大於5%

2. 吸入性燒傷(病史以及臨床症狀懷疑)

3. 電燒傷(病史及臨床症狀懷疑)

4. 化學性灼傷:面積大於10%體表面積

5. 燒燙傷患者合併危及生命之嚴重傷害或合併症者

6. TEN(Toxic Epidermal Necrolysis):表皮缺損大於30%體表面積

 

普通燒燙傷病房住院適應症

1. 急性燒燙傷患者

     甲、成人:二度以上燒燙傷面積介於10%~20%之間,三度以上面積小於10%

     乙、小兒:二度以上面積介於5%~10%,三度以上面積小於5%

     丙、二度以上燒燙傷面積大於1%,燒燙傷位於以下特殊部位:臉部、手部、會陰部、足部

     丁、二度以上燒燙傷面積大於1%:嬰兒(1歲以下)以及老人(70歲以上)

2. 慢性燒燙傷傷口癒合不良:燒燙傷傷口超過14天未癒

3. 燒燙傷傷口,需要光照治療者

4. 燒燙傷患者,因其他系統性疾病而影響傷口癒合者

5. 燒燙傷疤痕攣縮需手術重建者
